What is Phase 2 Cardiac Rehab?
Phase 2 Cardiac Rehabilitation is a medically supervised program designed for people recovering from heart attack, stent placement, bypass surgery, valve repair, stable chest pain, heart transplant or stable heart failure. This stage of rehab typically begins shortly after discharge from the hospital and provides the guidance, structure, and encouragement you need to safely rebuild your strength and confidence as well as decreasing risk of another heart event.
What to Expect in the Program
In our program, you'll attend structured exercise sessions in a safe, monitored environment where your heart rhythm, blood pressure, and progress is carefully tracked. Alongside exercise, we proved education on nutrition, stress management, and lifestyle changes that can help you reduce risk factors and maintain a healthy heart. Each session is designed to gradually improve your endurance, strength, and overall cardiovascular fitness. Enrolling in Phase 2 Cardiac Rehab is one of the most important steps you can take for your heart health.
Our Cardiac Wellness staff will guide you through each session and tailor your program as needed. When appropriate, your plan may also be coordinated with your healthcare team, including physicians, physical therapists, and family members.

Cardiac Wellness
The Rawlins County Health Center Cardiac Wellness Program offers a supervised, progressive exercise plan with scheduled one-hour sessions. A provider referral is not required.
Affordable monthly rates:
$35 for Monday, Wednesday, Friday sessions
$25 for Tuesday, Thursday sessions
Program Schedule:
Monday, Wednesday, Friday: 6:30 am - 5:00 pm
Tuesday & Thursday: 9:00 am - 10:00 am
The Cardiac Wellness Program is open to anyone, regardless of medical history. You may especially benefit if you have heart disease, diabetes, lung issues or general weakness. Many participants find the structured schedule helps them stay motivated and accountable.
